Kuisioner is an online survey platform that helps organizations create, distribute, and analyze surveys with professional templates and comprehensive analytics.

🌐 Guest/Public User Guide

Overview

As a guest (respondent), you can access and fill out surveys without creating an account. Surveys are either publicly available or access-restricted using tokens.

How to Access a Survey

Option 1: Public Survey (Direct Link)

  1. Receive Survey Link: Get a direct link from the survey creator

    • Format: kuisioner.id/{organization-slug}/{survey-slug}
    • Example: kuisioner.id/acme-corp/employee-feedback
  2. Open the Link: Click the link in your browser

  3. View Survey Details:

    • Survey title and description
    • Number of questions
    • Estimated time to complete
    • Organization branding (logo, colors, fonts)

Option 2: Token-Protected Survey

  1. Receive Token Link: Get a link with a unique access token

    • Format: kuisioner.id/{organization-slug}/{survey-slug}/token/{token-code}
    • Example: kuisioner.id/acme-corp/employee-feedback/token/abc123xyz789
  2. Open the Link: Tokens provide secure, one-time or limited access to surveys

    • Each token can restrict the number of attempts
    • Tokens can be tied to specific respondents
  3. Access Granted: You'll be directed to the survey automatically


Identifying Yourself (Optional)

If the survey requires self-identification:

  1. Fill Identity Form (if required):

    • Name: Full name
    • Email: Valid email address (for result notifications)
    • Phone: Contact number
    • Custom Fields: Any additional fields configured by the survey creator
  2. Click "Next" or "Continue" to proceed to survey questions

Note: Identity information is:

  • ✅ Optional for public surveys
  • ✅ Securely stored
  • ✅ Never shared with third parties
  • ✅ Used for respondent tracking and communication

Filling Out the Survey

Question Types You May Encounter

  1. Multiple Choice (Single Answer)

    • Select ONE option from the list
    • Radio buttons for selection
    • Example: "How satisfied are you?"
  2. Multiple Choice (Multiple Answers)

    • Select ONE OR MORE options
    • Checkboxes for selection
    • Example: "Which features do you use?"
  3. Likert Scale

    • Rate agreement on a scale (1-5 or 1-10)
    • Visual scale from "Strongly Disagree" to "Strongly Agree"
    • Example: "I find this product easy to use"
  4. Open-Ended (Essay)

    • Type free-form text responses
    • No word limit (unless specified)
    • Example: "Tell us your experience..."
  5. Ranking

    • Order items from best to worst
    • Drag-and-drop or select priority
    • Example: "Rank these features by importance"

Survey Features

Feature Description
Auto-Save Your progress is saved automatically as you type/select answers
Progress Bar See how many questions remain
Timer Some surveys have a countdown timer (if set by creator)
Back/Next Navigate between questions (if allowed)
Required Fields Marked with * (asterisk) - must be answered
Optional Fields Can be skipped or left blank

Filling Best Practices

Do:

  • Answer honestly for accurate results
  • Complete all required questions (marked with *)
  • Allow time for thoughtful responses
  • Use the auto-save feature (don't close abruptly)

Don't:

  • Refresh the page frequently (may lose unsaved progress)
  • Close the browser without submitting
  • Ignore error messages (red boxes mean required fields)

Submitting Your Response

  1. Review Your Answers:

    • Scroll through or use "Review" button (if available)
    • Verify all required fields are completed
  2. Click "Submit" or "Send Response":

    • Confirmation message will appear
    • "Thank you for completing this survey!"
  3. View Summary (if enabled):

    • Some surveys show results summary after submission
    • Or direct you to a thank-you page

Important: Once submitted, responses cannot be modified (unless survey allows multiple attempts).


Survey Attempt Limits

Surveys may have attempt restrictions:

Limit Type What Happens
Once Only You can submit the survey ONE time only
Unlimited Fill the survey as many times as you want
Custom Limit E.g., "Max 3 attempts" - creator sets the limit
Token-Based Each token allows a set number of attempts

Check Status: After submitting, the survey will tell you if you can attempt again.


Survey Response Limits

Some surveys have maximum response caps:

  • Example: "This survey accepts only 500 responses"
  • When limit is reached, survey closes automatically
  • You'll see message: "Survey is currently closed"

Troubleshooting for Guests

Issue Solution
"Survey not found" Check the link is correct; ask organizer for link
"You've exceeded maximum attempts" Survey limit reached; contact organizer
"Invalid or expired token" Token expired or invalid; request new token
"Required field missing" Fields marked with * must be filled
Progress didn't save Use auto-save; avoid closing unexpectedly
CAPTCHA verification Complete Cloudflare Turnstile puzzle to verify human

👤 Tenant Admin User Guide

Overview

As a Tenant Admin, you manage surveys, team members, and billing for your organization. Start by creating an account and then you can:

  • Create and publish surveys
  • Manage survey responses
  • View analytics and reports
  • Add team members
  • Upgrade your plan

Getting Started

Step 1: Create Your Account (Registration)

  1. Go to Registration Page: Visit kuisioner.id/register

  2. Fill Registration Form:

    Field Description Notes
    Organization Name Your company/organization name Required
    Admin Name Your full name Required
    Email Valid email address Used for login & notifications
    Password Strong password Min 8 chars, mix of letters/numbers
    Org Slug Unique URL identifier e.g., acme-corp
  3. Check Slug Availability:

    • The system checks if your slug is available in real-time
    • ✅ Green = Available
    • ❌ Red = Already taken (choose another)
  4. Accept Terms:

    • Read Privacy Policy & Terms of Service (links provided)
    • Check the agreement checkbox
  5. Click "Create Account":

    • Account created successfully
    • Verification email sent to your inbox

Step 2: Verify Your Email

  1. Check Email Inbox: Look for "Verify Your Email" message

  2. Click Verification Link:

    • Link is valid for limited time
    • Opens directly to your admin dashboard
  3. Account Activated: You're now ready to create surveys


Step 3: Login to Admin Area

  1. Go to Login Page: Visit kuisioner.id/admin/login

  2. Enter Credentials:

    • Email: Your registration email
    • Password: Your registration password
  3. Click "Login":

    • Redirected to admin dashboard
    • Displays your surveys, stats, and quick actions

Note: You'll see message if:

  • ❌ Tenant status is "Pending" → contact support
  • ❌ Tenant status is "Suspended" → contact support

Survey Management

Creating a Survey

  1. Click "Create Survey" button on dashboard

  2. Fill Survey Basic Info:

    Field Description
    Survey Title Name of survey (e.g., "Customer Feedback 2026")
    Description Brief description for respondents
    Survey Slug URL-safe identifier (auto-generated, editable)
    Response Template Visual theme (Edu-Fun, PSY-Calming, Professional, Default)
  3. Configure Survey Settings:

    Setting Options Purpose
    Access Mode Public / Self-Identified Who can access the survey
    Max Attempts 1 / Unlimited / Custom How many times respondents can submit
    Response Limit Unlimited / Custom Max total responses accepted
    Time Limit None / Custom (minutes) Countdown timer for responses
    Require Identity Yes / No Collect respondent info (name, email, etc.)
  4. Click "Create Survey":

    • Survey created and ready for questions
    • Redirected to Question Builder

Building Survey Questions

Adding Questions
  1. Question Builder Interface:

    • Shows list of existing questions
    • "Add Question" button to add new ones
  2. Select Question Type:

    • Multiple Choice (single answer)
    • Checkbox (multiple answers)
    • Likert Scale (agreement rating)
    • Essay/Text (open-ended)
    • Ranking (order items)
    • Short Text (1-line answer)
    • Long Text (paragraph)
  3. Fill Question Details:

    Question Text: "How satisfied are you with our service?"
    Help Text: "(Optional) e.g., 'Rate 1-5'"
    Required: ☐ Yes ☑ No
    Question Type: Multiple Choice
    
  4. For Multiple Choice / Checkbox:

    • Enter each option (e.g., "Very Satisfied", "Neutral", "Dissatisfied")
    • Can add/remove options dynamically
    • Can set option weights (for scoring)
  5. Save Question:

    • Click "Save Question"
    • Question appears in survey
Editing Questions
  1. Click Edit Icon next to question
  2. Modify Details:
    • Change question text
    • Add/remove options
    • Update required status
  3. Click "Update"
Reordering Questions
  1. Drag-and-Drop: Grab question by handle and drag up/down
  2. Or Use Arrow Buttons: ⬆️ Move up, ⬇️ Move down
  3. Changes Save Automatically
Deleting Questions
  1. Click Delete Icon (trash bin)
  2. Confirm Deletion: "Are you sure?"
  3. Question Removed: Cannot be undone (but can re-add)

Question Bank (Reusable Questions)

What is Question Bank?

Store frequently used questions for quick reuse across surveys:

  • Customer satisfaction questions
  • Employee feedback templates
  • Standard demographic questions
Using Question Bank
  1. Go to Question Bank: In Survey Editor sidebar
  2. Browse Saved Questions: List of reusable questions
  3. Add to Survey: Click "Add to Survey"
    • Question inserted at end
    • Can then edit or reorder
Saving Questions to Bank
  1. While Editing Question: Check "Save to Question Bank"
  2. Give it a Label: "Employee NPS", "Demographics", etc.
  3. Save: Question added to your bank
  4. Reuse Across Surveys: Available in all future surveys

Publishing a Survey

  1. Complete All Steps:

    • ✅ Basic Info filled
    • ✅ At least 1 question added
    • ✅ Settings configured
  2. Click "Publish":

    • Survey marked as active
    • Response collection begins
  3. Share Survey Link:

    • Public link: kuisioner.id/{org-slug}/{survey-slug}
    • Token links: Generated in Token Management
    • Copy and send to respondents
  4. Monitor Responses: Responses appear in real-time


Survey Configuration Options

Response Limits
Example:
- Max Attempts: 1 (respondents can submit once)
- Response Limit: 100 (survey closes after 100 responses)
- Time Limit: 10 minutes (countdown timer)
Identity Collection

If enabled, respondents must provide:

  • Full Name
  • Email
  • Phone (optional)
  • Custom Fields (if configured)

Response Management

Viewing Responses

  1. Click Survey Name on dashboard

  2. Go to "Responses" Tab:

    • Shows all received responses
    • Table with columns: Respondent, Date, Status, Score (if enabled)
  3. Filters & Search:

    • Filter by date range
    • Search by respondent name/email
    • Sort by column

Individual Response Review

  1. Click on Response Row: Opens response detail

  2. View All Answers:

    • Question text
    • Respondent answer
    • Time spent on question
    • IP address (if logged)
  3. Response Metadata:

    • Respondent name/email
    • Submission date & time
    • Time to complete
    • Device/browser info
  4. Actions:

    • ✏️ Edit (if allowed by settings)
    • 🗑️ Delete response
    • 📋 Print response

Response Status Indicators

Status Meaning
Completed Respondent finished and submitted
In Progress Respondent started but not finished
Partial Started; some questions skipped

Analytics & Reporting

Survey Analytics Dashboard

  1. Click Survey → Analytics Tab

  2. View Real-Time Metrics:

    Metric Shows
    Total Responses Number of completed submissions
    Total Attempts Includes partial/in-progress
    Completion Rate % of responses vs. attempts
    Avg. Time Average time to complete
    Response Rate (if token-based) % of tokens used
  3. Question-by-Question Breakdown:

    • Each question shows:
      • 📊 Chart (pie, bar, line)
      • 📈 Percentages
      • 🔢 Response counts
      • Top answer highlights

Scoring (Advanced Analytics)

If you've configured scoring rules:

  1. Go to Scoring Rules: Survey → Scoring Config
  2. Set Point Values: Assign points to each answer option
  3. View Scores:
    • Response list shows score for each respondent
    • Analytics shows score distribution
    • Export includes scores

Example Scoring:

Question: "How satisfied are you?"
- Very Satisfied = 5 points
- Neutral = 3 points
- Dissatisfied = 1 point

Exporting Responses

  1. Navigate to Survey Responses

  2. Click "Export" Button

  3. Choose Format:

    XLSX Excel spreadsheet with formatting

  4. Download:

    • File downloads automatically
    • Use for analysis, reporting, archiving

Token Management (Secure Access)

What are Tokens?

Tokens are unique codes that control survey access:

  • Limited Access: Only token holders can access
  • Attempt Limits: Each token allows 1, 3, or unlimited attempts
  • Tracking: Know who filled which survey
  • Expiration: Can set validity period

Creating Tokens

  1. Go to Survey → Tokens Tab

  2. Click "Generate New Token"

  3. Configure Token:

    Max Attempts 1 / 3 / Unlimited per token
    Respondent Name (Optional) Tie to person
    Email (Optional) Respondent email
    Expiration Never / Date specific
  4. Generate:

    • Token code created (e.g., abc123xyz789)
    • Copy or send to respondent

  • Email
  • SMS

Token Tracking

  1. View Token Status:

    • Who used the token
    • How many attempts used
    • Last used date
  2. Revoke Token (if needed):

    • Click "Revoke"
    • Token no longer works

Team Management (Admin CRUD)

Add Team Members

  1. Go to Settings → Team Management

  2. Click "Add Admin"

  3. Fill Details:

    Name Team member's full name
    Email Unique email address
    Role Owner / Admin / Editor
    Status Active / Inactive
  4. Send Invitation:

    • Email sent with login credentials
    • Team member sets password on first login
    • Access granted immediately

Admin Roles

Editor Create/edit surveys only (no responses access)

Managing Team Members

  1. View Team List: Settings → Team Management
  2. Edit Member:
    • Change role
    • Enable/disable access
  3. Reset Password:
    • Click "Reset Password"
    • New email sent with password reset link
  4. Remove Member:
    • Click "Remove"
    • Access revoked immediately

  1. Go to Settings → Branding

  2. Upload Logo:

    • Your organization logo
    • Appears on all survey pages
    • Max 500KB, PNG/JPG
  3. Customize Colors:

    • Primary color
    • Secondary color
    • Button colors
  4. Fonts:

    • Choose default font
    • Affects all surveys
  5. Custom Message:

    • Thank you message (shown after survey)
    • Header text
    • Footer text
  6. Preview: See changes in real-time

  7. Save: Changes apply to all surveys

Upgrading Plan
  1. Go to Settings → Membership

  2. View Current Plan: Status and usage

  3. Click "Upgrade to [Plan]"

  4. Payment Options:

    • 💳 Bank Transfer (Manual)
    • Select bank (BRI currently supported)
    • Upload proof of payment
    • Pending review (approved by admin)
  5. Activation:

    • After approval, new plan activated
    • Limits updated immediately
Checking Usage
  • Surveys created vs. limit

Logging Out

  1. Click Profile Icon (top right)
  2. Select "Logout"
  3. Redirected to Marketing Home: Login page available

Important: Logging out from any device logs you out everywhere (if security feature enabled).


Troubleshooting for Admins

Can't login Check email/password; use "Forgot Password" link
Verification email not received Check spam folder; request new verification
Survey won't publish Ensure all required fields filled, at least 1 question
Responses not showing Wait a moment (real-time sync); refresh page
Can't export responses Ensure responses exist; check file format selection
Token generation fails Check plan allows tokens (Professional+ plans)
Team member can't login Send password reset; check email in spam
Upgrade pending approval Contact support; provide reference number
